USPS Form 1583 is a required Postal Service document that authorizes our store to receive mail on your behalf. Federal regulations require all customers renting a private mailbox or using a commercial mail receiving service to complete this form before mail can be accepted.

After completing the form:
Print and sign Form 1583.
Gather your two forms of identification.(check section below)
Bring the completed form and IDs to our store.
Our staff will review your documents and finalize your mailbox setup.
Please note that we cannot begin receiving mail for you until all required paperwork has been completed and approved.
When submitting Form 1583, you must provide two valid forms of identification.
You will need:
One photo ID (such as a driver's license, state ID, passport, or military ID)
One additional form of identification showing your current address (such as a vehicle insurance, current lease, mortgage or deed of trust, voter card)
All identification must be current and valid.
